Clinic Appointment Systems Coordinator: WorcesterHealth Awareness is seeking a full-time bilingual (English/Portuguese/Spanish) Clinic Appointment Systems Coordinator in Worcester. Responsible for centralized appointment scheduling for four sites, providing information and/or referral to callers, filing medical records, computer data entry, maintaining the linked computer system for all our clinical sites, and performing other ancillary clinical duties. Involves working independently and as part of a health care team. Requires ability to sit for long periods of time, comfort working with people from diverse ethnic and cultural backgrounds and talking to people about reproductive health issues. Must have excellent telephone, interpersonal and organizational skills. Computer skills and the ability to multi task are essential. Degree or certification in secretarial office management/ health education/related field; work exp. may be substituted for education. HIV CLIENT SERVICES:HIV Case-ManagerHealth Awareness seeks an experienced staff to provide Case Management to clients living with HIV/AIDS in the MetroWest Area. The case manager is responsible for providing multiple aspectsof psycho-social care, conducting assessments/reassessments, & providing case management, harm reduction/risk assessment, positive prevention counseling, HIV, STD, & hepatitis education, referral to medical & needed services, advocacy & housing placement assistance. Own transportation required. Must be knowledgeable in STD’s, addiction, violence prevention, G/L issues & other related issues. Must have ability to deal with sensitive issues in racism, classism, homophobia & sexuality. Strong case management & client record documentation and charting skills. Experience working with persons living with HIV/AIDS. BA/BS in health education or Social Services related field preferred; work experience may be substituted for education. Bilingual candidates (English/Spanish) preferred. Minimum 3 years experience in human services with 2+ years working with HIV.This position is based at our offices at 354 Waverley Street, Framingham. AA/EOEHIV Peer LeaderHealth Awareness seeks a part-time employee as an HIV Peer Leader. The Peer Leader will facilitate weekly support groups as well as conduct one-on-one peer support with individuals living with HIV/AIDS. The support groups include a Men’s group, Women’s group, Latino group and a Monday Night Social group. The Peer Leader is responsible for sharing information and facilitating conversations on HIV treatment and adherence, treatment side effects, sexually transmitted infections (STI) and hepatitis education, harm reduction/risk reduction techniques, and overall health & wellness. The Peer Leader will also address issues such as isolation, stigma, disclosure mental health and substance use/abuse with Health Awareness clients, and will make referrals to our internal case management program for follow up, coordination and external referrals as needed. Own transportation is recommended. Must be knowledgeable about hepatitis, STIs, and HIV/AIDS prevention and health care education.
